Few things are as unpleasant as having an accident that lands you in the ER. The one exception might be getting hit with an unexpected or surpise medical bill.

But why are you getting a surprise medical bill if you are insured?

Here is What Happens

When you visit an emergency room, regardless of whether it is in-network or not; you may be treated by physicians and other medical staff that may not be part of your insurance plan network.

When this happens, the insurance companies won’t cover the full amount, and you will receive a bill saying you owe the out-of-network provider money. These bills are called balance bills and are commonly referred to as surprise medical bills.

What is Balance Billing?

If the emergency room, and doctor that treated you are both in-network, then you will only need to pay for what is negotiated in your plan, usually this involves:

  • Copay fees
  • Deductibles
  • Co-insurance amounts

However, if the ER or the doctor is an out-of-network provider, you will receive a balance bill, which is the difference between what the doctor charged and what the insurance company paid.

When you receive a balance bill or in other words a surprise medical bill, here are the steps you should follow:

 

 

1. Always Request an Itemized Invoice

 

Texas law requires providers to present an itemized bill upon request. Some providers will send it automatically, but if you don’t get it, ask for it!

Reviewing your bill could help reduce the amount owed in two ways:

  • You can verify there are no duplicate charges
  • You can confirm you are only being charged for procedures, tests, and treatments you received.

Billing departments handle vast amounts of data every day. So, it’s possible for some unintended errors to occur. Carefully reviewing your balance bill is the best way to avoid overcharges.

 

 

2. Call Patient Advocate

 

If you received a balance bill from us, the first thing you should do is contact our Patient Advocate.

Our staff is available to answer any questions and address any concerns you have regarding your bill.

Additionally, our Patient Advocate may be able to negotiate rates with insurance companies to help lower your bill.

We can even offer financial aid or customize a payment plan that works for you.

Surprise Medical Bills May Soon be a Thing of the Past in Texas

Earlier this month, in a unanimous vote, the Texas House approved Senate Bill 1264.

When signed into law, SB 1264 will prohibit out-of-network providers from invoicing patients for their services directly.

Instead, the law would allow providers and insurers to negotiate with one another to secure payment. The legislation also creates state regulatory authority over that arbitration process.

What this means for you as a Consumer

This is excellent news for you as a consumer!

This Senate Bill will give us more leeway to push insurance companies to pay fair prices for the services provided without having to burden you.

Emergency Room visits are often a disorienting, stressful, and nerve-racking affair, and to make matters worse, medical bills are notoriously difficult to understand, so your stress doesn’t end the minute you walk out of the ER.  Understanding your ER Bill is not something that is common knowledge.

Nevertheless, it’s in your financial best interest to know exactly what you are paying for and the ways you can lower your out-of-pocket expenses when you receive a medical bill.

At Altus Emergency Centers we believe in billing transparency, and helping our communities safeguard their hard-earned money. We know firsthand that there is no way to thoroughly plan for a medical emergency, which is why we are committed to help our patients receive all the benefits of their health plans and save as much money as possible.  For this reason, we bring you a simple guide to understanding your ER Bill that we hope will help settle your nerves and save you money.

What Are Facility and Physician Fees?

After receiving treatment in one of our centers, we will mail two separate claims to your insurance company:

  • Facility Fee Claim
  • Physician Fee Claim

 

Facility Fee

All Emergency Rooms (hospital and freestanding) charge a facility fee. Facility fee charges depend on the level of emergency care provided to our patients which ranges from basic emergency care to immediate life-threatening conditions.

The facility fee does not include medications, supplies, diagnostic imaging such as CT scans and X-rays, or laboratory tests.

 

Physician Fee

In the past, emergency physicians would bill for their services separately and their fees vary depending on the level of care provided.

Starting back on June 2019, at Altus Emergency we directly control physician billing to ensure you superior service.

All patients are treated exclusively by our in-house local doctors, and this allows us to control the entire billing process.  The best news is that you will never receive a surprise medical bill from us. Please keep in mind that physician services will still be billed separately for transparency purposes.

On the other hand, the physician fees of Radiologists (X-Rays) and Pathologist (Lab) are included in the facility fee.

Documents You Should Expect from Your Insurance Company

Once we file the insurance claims on your behalf, your insurance provider will review them and determine how much is covered by your plan and how much you need to pay out of pocket.

 

EOB

An Explanation of Benefits (EOB) is the statement sent to you by your health insurance company explaining which services your insurance covers, the amount paid by your plan and how much you need to pay out-of-pocket.

The balance due considers your deductible, coinsurance, and other non-covered benefits.

You should receive your EOB before you get an actual bill from us. When you get your EOB, please review it carefully, make sure you are not being billed twice for the same service or service you did not receive.

Double-check that the insurance company is paying for everything your plan includes, if you need help doing this you can contact our Patient Advocate, they can help you understand your coverage and negotiate with your insurance company.

 

Informative Letters

Some insurance companies will send you a letter with additional information regarding your coverage. They will appear to sound “nice” while they explain that they are not covering your costs and even claim that they will try to help you lower your out-of-pocket costs.

Some of these letters might make it look like independent ER’s (also called freestanding ERs) like Altus Emergency Centers are more expensive than hospital-based ER’s.

Check out an example of a real letter from Cigma that an Altus Emergency patient received after being treated.

However, thanks to a recent law which forces medical providers to list their standard costs for procedures we now know this is not true.

In fact, we can prove that Altus Emergency is not more expensive than a hospital ER.  In some cases, we are actually less expensive than a hospital ER and we can PROVE IT.

You know what they say about sticks and stones?  Well, a broken bone really hurts!  Bookmark this easy guide of the signs of a broken bone to know what to do if you or someone you love ever has an accident in the future.  Bone fractures, like most other injuries, vary in severity, and diagnosing is not easy without the use of advanced imaging technology like X-rays.

Some folks believe they can tell if a bone is broken by whether the patient can use it or put weight on it or not. Unfortunately, that is a myth; it’s surprising how often people who have a broken bone don’t even realize it because they can move it.

That is why it’s important to know the signs of a broken bone and get the issue fixed before it becomes an even bigger problem.

Signs of a Broken Bone

1. Bruising and Discoloration

Most of us tend to ignore our bruises thinking they are not that big of a deal, but they could be the first sign that your fall or accident led to a broken bone.

Bruising occurs when tissue capillaries get damaged, and blood leaks out of them. The bigger the bruising area is, the more likely damage involves deep tissue, possibly bone deep.

 

2. Swelling

 

There are many reasons behind swelling, but when it occurs after a trauma, it could be a sign of a broken bone. Swelling occurs when fluids and sometimes blood from an injury start to leak into the surrounding soft tissue like muscle, fat, and skin. The extra fluids cause the tissue to swell and become hard and painful to touch.

 

3. Deformity

 

If your injured arm, leg, hand, or foot look deformed, chances are you broke it. If your arm or leg bends in strange ways, this is a clear indicator that you need to come into the nearest Altus Emergency Center to get this issue fixed.

4. Crepitus

 

This word sound ominous and it is. Crepitus is when you can feel or hear crunching beneath your skin. Trust us when we say, that’s not supposed to exist anywhere in your body; and if it does, then this is a sign there are broken pieces of bone rubbing against one another.

 

 

5. Loss of Function

 

Loss of function is an indicator that something is not right, and your injury could have caused a bone to break. If you cannot move your arm, hand, leg, or foot, then you need to be seen by one of our doctors as soon as possible.

Remember being able to walk or move your injured limb does not mean you do not have a fractured bone if you are in severe pain and are experiencing any of the above signs you need to be seen by a doctor as soon as possible.

Know Where to Go in Case of an Emergency.

When to Go to the ER

You should rush to the nearest Altus Emergency Center if you experience any of the following after a fall or other trauma:

  • Heavy Bleeding at the site of the injury
  • Numbness in the injured or surrounding area
  • The area turns a bluish color
  • Severe swelling
  • A visible deformity at the injury site
  • Bone protruding through the skin (Open fracture)

Dislocations

You should also come in if you have a dislocated joint. In many ways, a dislocated joint can be even worse than some types of fractures. Dislocations are serious orthopedic injuries, and they happen when a joint such as a knee or shoulder become severely misaligned.

Dislocations always involve deformity and can cause stretching or even tearing of ligaments and tendons. These types of injuries will always require emergency medical attention.

 

The only sure way to know if you have a broken bone or another type of orthopedic injury is with advanced imaging tests like X-rays, MRI’s, and ultrasounds.

How to Keep Your Kids Healthy

April was a fantastic month, but it’s not over yet, as we near the end of this month we still have one significant observance in sight, Every Kid Healthy™ Week!

This annual observance shines a spotlight on the great efforts’ schools are making to improve the health and wellness of our kids and the link between nutrition, physical activity, and learning.

But despite all the efforts schools are making, there is no denying that children in large groups have a knack for spreading germs and viruses that cause illnesses. For this reason, we are sharing five essential health tips to help make sure your kiddos stay healthy.

#5Teach them to Wash Their Hands Frequently

Kids are always playing, and as a result, their hands touch a lot of surfaces that could be dirty or harbor bacteria, viruses, and germs that could make them sick.

Teaching your child how to wash their hands properly is a great way to minimize their exposure to harmful organisms.

The CDC’s guidelines for washing hands include the following steps:

  • Wet hands thoroughly with water (temperature does not matter).
  • Lather with soap and make sure your kids apply soap to the back of their hands as well.
  • Instruct them to scrub their hands for 20 seconds or more, make sure to rub the back of their hands, wrists, in between fingers and under the fingernails.
  • They should rinse with plenty of water.
  • Teach them to always dry their hands thoroughly with a clean towel or an air dryer

#4 – Feed Them Healthy Meals and Snacks

A healthy diet is one of the best ways to prevent the onset of chronic diseases like:

  • Obesity
  • Heart Disease
  • High Blood Pressure
  • Type 2 Diabetes

Like most other habits, healthy habits are more likely to stay with you if you learn them as a kid. That’s why it’s important to teach your kiddos good habits now.

We understand everyone is busy these days and eating every meal at home is not always possible. And let’s be honest there is no sure way to prevent your kids from eating junk food.

You shouldn’t stress over this and don’t ban your children from eating a burger or pizza when they go out with friends. Instead of prohibiting your kids from eating certain foods, focus on the 80-20 rule. Try your best to make sure your children eat healthy 80% of the time and accept that the other 20% of the time, they will be eating an entire bag of potato chips all by themselves.

#3 – Keep Them Active

Exercise is vital for bone and muscle health, alleviating stress, creating a positive self-image, and preventing diseases later in life.

Most children need a minimum of 1-hour of physical activity per day, sports, dancing, swimming, are just some examples of fun ways for your kids to reach that goal.

However, before you send your kid off to the practice you need to make sure they will be safe, sports injuries are common. 

Learn to recognize the signs of a severe injury and when to bring your child to the ER.

 

Orthopedic Injuries

Bring your child to our pediatric ER when:

  • Your child cannot use or move their arm correctly, or they can’t bear weight on an injured leg
  • The limb that was injured looks misshapen, or bone is protruding from the skin

 

Concussions

A concussion is a serious brain injury, especially for a child, you should bring your kid to our pediatric ER as soon as possible if they have any of the following symptoms:

  • Loss of Consciousness
  • Change in Level of Consciousness (asks repetitive questions, has memory loss)
  • Vomiting (more than once)
  • Fever
  • Abdominal Pain
  • Neck Pain
  • Difficulty Breathing

#2 – Teach them to Keep Hydrated

It’s easy for kids to forget to drink enough water when they are playing, and this makes it easy for them to become dehydrated.

A great way to teach your kids to stay hydrated is to have them look at their pee, the clearer it is, the better, show them that if their pee is dark yellow, they should drink a glass of water or two.

Children with mild dehydration can be treated at home, provide them plenty of fluids and fruits and they should recover well. However, for kids who have moderate to severe dehydration, it’s important to get them to the nearest Altus Emergency pediatric center for treatment.

 

Signs and Symptoms of Moderate to Severe Dehydration

  • Dry, cracked lips and a dry mouth
  • Passes less urine than usual, no urine for eight to 12 hours, or dark-colored urine
  • Drowsiness or irritability
  • Cold or dry skin
  • Low energy levels, seeming very weak or limp
  • No tears when crying
  • Sunken eyes or sunken soft spot (fontanelle) on baby’s head

Before we discuss the Cost of an ER, let’s put everything into perspective.

How do you know when the price of something is worth its value?

Think about a situation when you felt whatever the price you paid for something wasn’t worth it, regardless of how cheap it was.

My “Aha!” moment happened when I rented my first place and had to go grocery shopping for the first time, I was on a tight budget, and bargain hunting seemed like the reasonable thing to do. I went as cheap as I could and felt proud of how much I had saved. While I admit some things really didn’t have much of an impact in my life, the old supermarket sushi certainly did.

When the expired sushi kept me in the bathroom all day, I understood that there are some things you can’t go cheap on:

  2. Toilet paper
  3. House Paint
  4. Tattoo
  5. Your Lawyer
  6. Fixing Your Car  
  7. Hiring an inexpensive baby sitter

The other problem we have is,

we spend indiscriminately on things we don’t need to survive.

Things like watches, expensive sneakers, video games, outfits we may never wear but which must have matching shoes and purses just in case we ever do wear them. Yet, we squirm at the thought of paying for quality healthcare, especially when the cost of an ER visit comes to mind.

Where to Go in Case of an Emergency

Urgent Care clinics are a good option when you have a minor medical emergency, like if you cut your finger while cooking and need a couple of stitches. However,

Get to know Kim Gries, she’s here to help you get the most from your insurance coverage.  As a mother of two, Kim understands firsthand how important it is for Texas families to get hands on assistence when processing insurance claims.  We had the pleasure of chatting with Kim and she shared about working at Altus Emergency and her lovely family.  The following are highlights from our interview.

Get to Know this Houston Native

Q: “Tell us a little about yourself.”

A:  Kim.

I was born in Houston, Texas and moves to Lake Jackson at the age of 12-ish. Growing up I was always a tomboy so fishing, roller blading and skinned knees were a usual thing for me. Having kids of my own now, I can only hope they have as much fun as I did, doing those sorts of things!

Today as an adult, my favorite things include gardening, reading, watching my kids play and spending time with my husband.

Q:  “What about your studies and experience?”

A:  Kim.

I was fortunate enough to attend Texas School of Business in 2007 with studies specializing in medical assisting. That ranges anywhere from working with medical insurance in any medical setting and front office work to blood draws and EKG’s. I really enjoy being on the clerical side, however.

I did my internship in a Neurosurgeon’s office where I was hired on and stayed for almost 9 years to be the surgery scheduler and help resolve patient concerns with insurance preauthorization and verification.

Q:  “Why did you go into healthcare billing?

A:  Kim.

My mother worked as a medical biller for many years in a doctors office and it was always a familiar setting for me (ie. Take your kid to work day) so after high school it only felt natural to follow suit. It’s been interesting to say the least as everyday brings something new and enlightening to the table.

Q: “When did you start at Altus Emergency?

A: Kim.

I began working at Altus Emergency Center in October 2018 at the registration desk. I am extremely thankful of this because I feel better rounded and have a better understanding of the entry and billing process of each patient that comes in. This I believe helps me see all angles better and lets me assist patients at the best of my abilities.

Q: “What do you love about being a part of the Altus Emergency Team?

A: Kim.

The thing I love most about working at Altus is the people. Everyone here is so friendly and always willing to lend a helping hand or ear for that matter. It’s a well-oiled machine and everything runs so smoothly on a daily basis. You can really appreciate that kind of setting after being a stay at home mom for almost 3 years!

Q: “Last of all, what are your biggest accomplishments?

A: Kim.

I’d say my biggest accomplishment would be overcoming some really hard times my husband and I have been through in the last few years, staying a strong married, loving couple and enjoying our 2 young children.

What to Do When You Can’t Afford Your ER Bill

Regardless of how organized you are with your money, nobody is ever fully financially prepared to deal with a medical emergency.

In recent years, most American’s have developed a real fear of having to visit a hospital or emergency room.  According to a study done by Harvard University, medical expenses were responsible for 62% of total bankruptcies in the US with 72% of those who filed bankruptcy due to medical costs had some form of insurance. If you are insured, you should be covered, right?  The fact is insurance companies with do everything possible to avoid paying claims.

With the growing trend of insurance companies looking for unlawful ways to deny emergency room visit claims, what can you do to avoid financial distress?

1. Don’t Panic!

We know having to deal with unexpected medical bills is a source of dread and anxiety, but, the worst thing you can do once you receive a bill from the emergency room is to panic.

Allowing anxiety to take over can cause more problems for you down the road. Keeping a clear, calm attitude will let you to review your bill and figure out what your next step should be.

2. Review the Bill for Errors

Calmly check the bill to make sure you are being billed for the treatment you received; there have been cases where a patient was double billed for the same service by mistake, or even being charged for a procedure they did not receive.

  • If your bill does not include a detailed description of the charges, call the provider and ask them to send it to you.
  • It’s important to know that healthcare providers use codes known as Current Procedural Terminology (CPT) to report medical, surgical, and diagnostic procedures and services, you can search for them on the internet to decode their meaning.
  • Make a list of any items you have questions about.
  • Call the billing department at Altus Emergency Centers to make an appointment to review your bill as soon as possible. If there is one, they will be corrected, and a new statement will be sent to you.

3. Contact Your Insurance Company

If you have health insurance and feel you should not have to pay the amount owed, you need to contact your insurance agent as quickly as possible to ask why they are not honoring the terms of your insurance plan.

  • If your insurance company says the CPT code on the insurance claim is not covered by your plan, ask them which codes are covered by the program. This is important because in some cases our billing department might be able to change the code for a similar one that is covered by your insurance plan.
  • When your claim was denied because your insurer did not consider your procedure to be medically necessary, our doctors would be happy to write a letter explaining why the course of treatment was required.
  • If the insurance company is refusing to pay for your emergency room visit because the ER was not in network, let them know you are aware of your rights as a patient, and that the law requires insurance companies to pay for emergency medical treatment regardless of whether the facility is in network or not.

4. If All Else Fails, File an Appeal

There will be times when your insurance company will try to use fear tactics to get you to back down and pay out of pocket for services that should clearly be covered in your plan.

If you have tried to reason with them, and it has not worked, contact the Patient Advocate team at Altus Emergency Centers and bring them up to speed with everything you have done up to that point. They will try to speak to your insurance provider, and if they are unable to resolve the issue, they will gladly file an appeal on your behalf.

If your formal appeal is denied, you can refile another, and if the insurance company declines that one too, then you can request your claim to be reviewed by an independent third party who will give a final resolution.

5. Negotiate

If all your appeals are denied, or you are uninsured or are unable to pay the out of pocket amount, there are options available for you.

At Altus Emergency Centers we offer financial assistance for patients who cannot afford to pay their bills at once.

The first thing we will recommend is that you request a payment plan rather than use your credit card to pay for the amount due. Our Patient Advocate personnel can design a payment plan that fits your budget.

 

While it’s always a good idea to save for a rainy day, a medical emergency can happen at any time and to anyone. Don’t Fall Prey to the Insurance Company Fear Campaign

A study from the Doctor Patient Rights Project (DPRP) has revealed a disturbing trend amongst large insurance companies.

In the study, the DPRP singled out a company named Anthem Blue Cross – Blue Shield, which through its affiliated networks is the nation’s largest private health insurer. The findings in the study claim that Anthem has been retroactively denying insurance claims for emergency room visits from its clients, especially those who are from low income or rural areas.

The DPRP concluded that the purpose of Anthem’s policy is to make its clients too afraid to go to the ER out of fear that they will not be covered and must pay the whole bill out of pocket, which most patients are unable to do.

This conclusion is widely shared by the physicians and administrative staff of Emergency Rooms across the country as put in evidence by the comment of Dr. Ryan Stanton a critical care and emergency medicine specialist in Lexington, Kentucky,

“The purpose of this program is to spread fear.”

What is Anthem Blue Cross – Blue Shield Doing?

Last year, Anthem started notifying its clients in certain parts of the country, that if their ER visits end with a diagnosis for something that isn’t an emergency, they would be responsible for 100% of the cost of their visit.

The problem with this policy is that most of us don’t have enough medical knowledge or training to self-diagnose before going to the ER. According to The American College of Emergency Physicians (ACEP), there is nearly a 90-percent overlap in symptoms between emergencies and non-emergencies, according to a study in the Journal of the American Medical Association (2013). These symptom overlaps make it extremely difficult for any of us to determine the difference between abdominal pain that is life-threatening and abdominal pain that isn’t.

The fear factor that Anthem is creating with their denial policy means more people will choose to wait and see if their symptoms improve before seeking immediate emergency medical attention, a decision that could lead to lifelong disabilities or even death.

The Law is on Your Side

The policy is so outrageous and dangerous to the public, that it has even reached the Senate. As reported by CBS News, Sen. Claire McCaskill, D-Missouri, sent a letter to both the U.S. Department of Health and Human Services and the Department of Labor asking them to investigate whether certain health insurers had violated the Prudent Layperson Standard.

The Prudent Layperson Standard which was added to the group and individual health insurance plans in 2010, and it requires health insurance companies to cover emergency room visits based on symptoms and not the final diagnosis. It also eliminates the requirements for prior authorization before seeking emergency care.

In our state, the Texas Department of Insurance requires insurance companies to pay emergency facilities like Altus Emergency Centers, the same way they would bill a facility which is part of the insured’s in-network. In other words, your insurance company should cover the same amount regardless of what type of emergency facility you visit, they cannot provide you with a lower benefit level.

The Patient Protection and Affordable Care Act also added numerous patient protections, most notably, they now require health plans covering emergency services to provide such coverage without the need for prior authorization.

Furthermore, health plans are required to pay for emergency visits for medical situations in which an average layperson believes his or her health is threatened. The final diagnosis should not influence whether the insurer pays for the emergency room visit or not, and insurance companies cannot legally apply the claim towards the out-of-network benefits.

What Can You Do If Your Claim is Denied?

The first thing you should do is contact our Patient Advocate Department. Our staff will review your denial and process an appeal on your behalf; they will submit documentation and justification for any ER treatment that you received.

If you get denied a second time, you still have the option to request an external review by a third-party, which will most likely be the state insurance regulator.

The appeal process is lengthy and labor-intensive because the goal of the insurer is for you to give up. Regardless of how long it takes you should continue with the process because the law is on your side.

Don’t fall victim to these scare tactics being implemented by insurance companies.
